What does the first appointment for couples involve?

The first appointment for couples is a medical consultation during which the doctor will take a medical history from you and your partner and assess your health to gather information about your fertility. To ensure your first appointment is as effective as possible, you should prepare properly for your consultation with the doctor. A midwife or Patient Care Coordinator from the INVICTA Clinic will contact you to help you gather the necessary documentation, test results and medical history (if you and your partner have previously been patients at another clinic). It is important that you both attend the first appointment together. Remember that difficulties in conceiving are not only influenced by your own health – the male factor is equally important. The effectiveness of treatment for fertility problems depends on early and comprehensive diagnosis of the couple.

During your first consultation, based on the information gathered, the doctor will attempt to diagnose the cause of your difficulties in conceiving. There can be many causes of fertility problems, and their underlying causes may vary. Hormonal disorders and ovulation disorders are among the most common causes of infertility.

When should you seek advice from a specialist?

Every situation is different, so it is important that you listen to your body and decide to consult a doctor when you feel there are reasons for concern. However, it is generally accepted that women under the age of 30 should see a specialist after a year of unsuccessful attempts to conceive, whilst women over the age of 30 should do so after just six months of unsuccessful attempts.

How should I prepare for my first visit to the INVICTA Clinic?

  • We will send a questionnaire – containing a detailed medical history – to the email address you provide. It is worth completing this before your appointment to save time at the Clinic.
  • Before your appointment, you will receive a set of useful information and a list of diagnostic tests that need to be carried out prior to the consultation, so that the specialist can analyse the results straight away during your appointment.

  • With your test results and medical records from your previous treatment, we will invite you to an in-person appointment at the INVICTA Clinic or an online appointment at the Virtual Clinic.

  • The next stage is a consultation with a specialist in infertility treatment.

What can you expect during your first visit?

Step 1

Medical history

The doctor will ask questions about your health history, past illnesses and any operations you’ve had. They will be particularly interested in your medical history as it relates to fertility, such as how long you’ve been trying to conceive and what your menstrual cycles are like. The doctor will also want to know about your partner’s health and fertility history, as both partners can influence your ability to conceive.

Step 2

Diagnostic tests

Based on the information obtained during the consultation, the doctor may recommend further diagnostic tests to help identify the possible causes of fertility problems. These may include hormonal tests, imaging tests (e.g. ultrasound), a semen analysis and genetic tests.

Step 3

Analysis of research findings

Once the tests have been carried out, the doctor will be able to interpret the results and explain whether there are any clear reasons for the difficulty in getting pregnant.

  • Yes, because the cause of fertility problems very often lies with both partners.

  • During your first appointment, the doctor will carry out a detailed medical history review and may also request blood tests, an ultrasound scan and a semen analysis.

  • AMH (Anti-Müllerian Hormone) is a hormone produced in both women and men. In women, AMH is produced in the ovarian follicles by the cells surrounding the eggs ready for growth. AMH levels are not influenced by a woman’s menstrual cycle, so they can be tested on any day of the cycle. AMH levels decrease as a woman ages, making it a very good marker of declining fertility, including premature ovarian failure. In patients with PCOS (polycystic ovary syndrome), AMH levels are significantly elevated.

  • Inhibin B is used to assess ovarian reserve. Inhibin B is produced by the granulosa cells of early antral follicles. This substance is secreted primarily during the follicular phase of the menstrual cycle. Its concentration in the early follicular phase reflects the number and quality of ovarian follicles. There is a positive correlation between inhibin B concentrations and the number of antral follicles assessed by ultrasound in the early days of the menstrual cycle.

  • The aim of assessing ovarian reserve is:

    • to identify young patients with a diminished ovarian reserve and to initiate treatment at an earlier stage,
    • to identify middle-aged patients who still have viable oocytes and to determine the appropriate clinical management for them,
    • to rule out patients who have no chance of achieving pregnancy (due to the high costs and burden of treatment).
  • Laboratory tests, when carried out and interpreted correctly, are an essential diagnostic tool. The basic diagnostic panel for women at INVICTA Clinics comprises 12 laboratory tests.

    The tests included in the basic diagnostic panel must be carried out on an empty stomach. They are performed using a blood sample taken from the patient at all our centres. You should allow approximately 2 hours for all the tests to be carried out and for the paperwork to be completed in preparation for your first consultation with a specialist.

  • The man should have a general semen analysis carried out. He should abstain from sexual activity for 2 to 7 days prior to the semen analysis. The sample (collected via masturbation) can be provided at the INVICTA Clinic, in a private room (subject to prior booking), or delivered to the Clinic within 30 minutes of collection. The semen is submitted for analysis in a sterile container (such as a urine sample container).

    A semen analysis is the primary indicator of male fertility. It enables us to assess a man’s reproductive potential and accurately diagnose the underlying causes of any fertility issues he may have.

Fertility. What else is worth knowing?

Health problems or underlying conditions can affect your ability to conceive and the course of your pregnancy. It is worth knowing how other conditions can affect fertility and what precautions you should take when trying for a baby.

  • Conditions such as thyroid disorders, polycystic ovary syndrome (PCOS) and diabetes can affect hormonal balance and the menstrual cycle, which may make it difficult to conceive.

  • Hashimoto’s disease is a chronic lymphocytic thyroiditis which can develop asymptomatically over many years.

  • It is a condition in which tissue similar to the lining of the womb grows outside the womb. The initial symptoms of endometriosis include, amongst others, increasing pain in the lower abdomen and sacral region during menstruation, as well as painful sexual intercourse. Endometriosis can cause pain and disrupt the menstrual cycle, as well as making it difficult to conceive.

  • Anatomical disorders of the reproductive organs, such as adhesions in the ovaries or damage to the fallopian tubes, can hinder sperm motility and fertilisation.

  • Certain infections, such as urinary tract infections or sexually transmitted infections, may affect a woman’s ability to become pregnant or pose a risk to the course and continuation of a pregnancy.

  • High blood pressure, heart disease, asthma or autoimmune disorders can make it difficult to conceive.

  • Some medicines, particularly those taken on a long-term basis, may affect fertility and pregnancy. It is worth discussing with your doctor whether the medicines you are taking are safe whilst trying for a baby and during pregnancy.

  • Some genetic conditions can affect a person’s ability to conceive and may be passed on to their children.

  • Cancer treatments, such as chemotherapy, radiotherapy and certain medicines, can have a negative impact on the ability to become pregnant, and even on ovarian function. In some cases, treatment may cause temporary or permanent infertility, so it is worth considering options for preserving fertility before starting treatment.

  • Infertility may be caused by problems with the ability of male sperm to fertilise an egg.

Every couple is different, and the impact of underlying health conditions on fertility can vary depending on individual circumstances.
